By Entering this Attraction you agree to the following
This haunted attraction reserves the right to refuse admission to anyone.
You may experience intense audio and lighting, extreme low visibility and darkness, strobe lights, fog, damp or wet conditions, special effects, sudden actions, and an overall physically demanding environment.
You should NOT ENTER the haunted house if you suffer from asthma, heart conditions, are prone to seizures, have physical ailments, respiratory,
or any type of medical problem, or are pregnant or suffer
from any form of mental disease, including claustrophobia.
DO NOT ENTER the haunted house (or enter at your OWN RISK) if you are intoxicated, wearing any form of cast, medical brace, using crutches, or have any type of physical limitations. Do not enter the attraction if you are taking medications or using drugs of any type. You will not be admitted if any of these conditions are to be noticed by our staff.
DO NOT ENTER THE HAUNTED HOUSE IF YOU SUFFER
FROM ASTHMA, HEART CONDITIONS, SEIZURES, OR ANY TYPE OF MENTAL,
PHYSICAL, RESPIRATORY, OR MEDICAL PROBLEMS.
DO NOT smoke, run, eat, flashlights, or drink inside the Haunted Attraction. Additionally, there is NO TOUCHING of actors, customers, or props inside the haunted house. No video or flash photography may be taken inside the Haunted Attraction. You will not be admitted and will be asked to leave the property if any of these rules are not followed.
UZA MysCreation reserves the right to refuse admission to anyone!

Warning!! You going in is a license to enter at your own risk. All patrons assume all risk and danger incidental to UZA MysCreation before, during, and after the tour. You may/will be videotaped and photographed and your images may be broadcasted on our website with no compensation. By entering UZA MysCreation YOU waive any injury or talent claims what so ever and accept these terms and conditions. Specifically, you claim no copyright interest in your image if it is used as part of any websites,
any video images, or any promotional materials for UZA MysCreation,
United Zombies of America, UZA Synyster Syn, OddIty or others.
You hereby release all broadcast and talent rights.

This Haunt Seasons to enter UZA MysCreation Haunted Attraction you must practice social distancing, wear a maskis optional, wash your hands, can't have a Temperature over 103 F-104 higher, can't have any Covid-19 symptoms. NO Touching of Cast, Crew, Props just
keep your hands to yourself, please!
If any of these rules are broken
no entering and may be escorted away
from the Haunted Attraction.
We have the right to refuse service to anyone.
I acknowledge the contagious nature of the Coronavirus/COVID-19 or any Flu or illness and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19 or any Flu or illness.
I further acknowledge that UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any Flu or illness may result from the actions, omissions, or negligence of
myself and others, including, but not limited to,
UZA MHA Scream Team and UZA Team,
and other patrons and their families.
I voluntarily seek services provided by MysCreation Haunted Attraction and United Zombies of America UZA LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19 or any Flu or illness. I acknowledge that I must comply with all set procedures to reduce the spread while
attending Attraction/Event.
I attest that:
* I have not traveled internationally within
the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 or any Flu or illness.
* I have not been diagnosed with Coronavirus/Covid-19 or any Flu or illness and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19 or any Flu or illness.
I hereby release and agree to hold UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Attraction/Event, or that may otherwise arise in any way in connection with any services received from UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC. I understand that this release discharges UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC from any liability or claim that I, my heirs, or any personal representatives may have against the Attraction/Event with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from UZA MysCreation Haunted Attraction and United Zombies of America UZA LLC.
This liability waiver and release extends to the
Attraction/Event together with all owners, partners,
and UZA MHA Scream Team and UZA Team.
